Birthplace & country of chargeability
Use the cycle’s official country list. Citizenship and residence do not normally replace birthplace. Spouse/parent chargeability exceptions and territory rules need individual assessment.
Education or qualifying work experience
The principal applicant needs qualifying formal schooling or qualifying work experience. The education route requires completion comparable to a U.S. 12-year primary/secondary course. O*NET work qualification uses Job Zone 4/5 and SVP 7.0+.
Valid passport & entry scan
Current regulation requires valid passport details and a scan of the biographic/signature page, subject to limited legal exemptions. Scan format: JPEG/JFIF, maximum 5 MB; not PDF.
Names, birth details & contact information
Enter complete names in the required Roman-letter fields, correct birth date/place, current mailing information and an email you control. State your completed education level accurately.
Spouse, children & separate family photos
Include required spouse and unmarried under-21 biological, step and legally adopted children, including those not traveling. U.S. citizen/LPR and court-ordered separation exceptions apply.
One entry, entry period & confirmation number
Duplicate entries for one person in the same cycle invalidate their entries. Each qualifying spouse may enter independently. Submit only during the official window and keep your confirmation number.
Service charges versus government fees
LotteryUSA’s $5 is an independent preparation-service price. The government fee schedule currently lists a $1 principal registration fee and a $330 DV application fee per visa applicant. Medical and other charges are separate.
